Case Management
We provide intensive Case Management services to people with severe mental illness aged 16 or older living in St. Thomas or Elgin County who need help working on skills for living independently. This service is provided through our main office in St. Thomas and through satellite offices positioned in the east and west of the County. Case Managers provide advocacy and one-to-one help and support to people learning how to live with and manage their mental health. Case managers can assist with improving relationships, managing money, getting a job, finding housing, and accessing addictions or other support services.
Services are client-directed and are based on individual need. Services are provided using a psychosocial rehabilitation model focused on recovery. Hours are Monday through Friday, 8:30AM to 4:30PM. Additional support is available through our Crisis & Intake team.
